Credit Card on File Policy

Starting 12/1/22, we will require patients to have a credit card on file with our office. This will allow the billing and payment process to be much simpler for you and our team. Nothing is changing about how much you pay and we still accept the same insurance plans we always have.  

Due to the high number of deductible plans, and higher patient coinsurance requirements, this has become necessary at our practice. Please keep in mind, we will not charge your card if you do not owe anything.  We have chosen to do this to collect only what is owed for our services.    

Your credit card on file will be used to pay account balances after your insurance pays and determines your responsibility. Once your insurance has processed your claims, they will send an Explanation of Benefits (EOB) to both you and our office showing what you owe, if any.   

You will be notified via email, text or letter (based on your preference) 5 days prior to your card being charged should you wish to change the card or dispute the amount. You will only be charged the amount indicated on your insurance Explanation of Benefits.  

Please note that we do not store your sensitive credit card information in our office. Once the card is swiped, only the last four digits of your account number is visible to our staff. Your account information is stored on Elavon, a secure, PCI and HIPPA compliant credit card processor and is only accessed to process your payment and email you a receipt once the payment is processed.

No-Show & Late Appointments for Primary Care & Sports Medicine

Complete Health Partners has made a commitment to you, our patients, to see you as close to your scheduled appointment time as possible. In fact, it is one of our core values as a company.

Due to the incredible response we have received for primary care and sports medicine, our providers schedules have rapidly filled and new patients are occasionally experiencing a delay in scheduling an appointment. When a patient does not show up for a visit or cancels/reschedules on the day of the appointment, it creates a hole in the schedule that can’t be filled. This is not fair to the patients who are forced to wait because the schedule was full.

Many practices will over-book or double-book patients throughout the day to account for this. Invariably, this leads to long wait times, extremely rushed visits, or sometimes both. We at Complete Health Partners place a high value on your time, and we know that longer, less rushed visits are essential to good primary care and sports medicine. In order to reduce the number of no show or late change visits, we are implementing a no-show/same-day cancellation fee.

Effective February 1, 2021 we will be implementing a no-show/same-day cancellation fee for primary care and sports medicine appointments. This fee would apply for same-day rescheduling as well, unless there was another appointment available for the same day that you are able to attend.

The fees are as follows:

  • No Show: $50
  • Same day cancellation $50
  • Same day reschedule (unless able to reschedule for different apt same day) $25

The fee will need to be paid before your next visit can take place and will not be billed to insurance.
